Summary of Benefits July 1, 2005
Flexible Benefits Program :
Health Insurance (includes prescription program): BC/BS Comprehensive $200 (Traditional) BlueChoice ((POS) Matthew Thornton Blue (HMO)
Dental Insurance: Delta Dental - options for coverage A through D
Life Insurance: Mutual of Omaha Life Insurance - 1 times salary, with optional insurance available in $10,000 increments, not to exceed 2 times salary.
Short Term Disability: Town - 60% of base pay after 30 days for up to 90 days
Long Term Disability: Mutual of Omaha - 60% of base pay after 90 days.
Employee Reimbursement Accounts: Health Care Account - up to a maximum of $2,500 Dependent Care Account - up to a maximum of $5,000
Supplemental Insurance: AFLAC - Personal Accident Insurance, Personal Cancer Protection Plan, Voluntary Indemnity Plan, Personal Hospital Intensive Care Insurance.

Earned Time (for exempt employees): Accrued on the basis of years of service 34 days - first five years 39 days - after five years 44 days - after ten years Maximum accumulation is 170 days

Leave:
Vacation - Accrued based on years of service for non-exempt employees. 2 weeks during first five years 3 weeks after five years 4 weeks after ten years
One additional vacation day granted for not using sick leave in any four-month period.
Vacation leave may be accumulated up to certain maximums based on years of service. 4 weeks during first 5 years 6 weeks after five years 8 weeks after ten years
Sick Leave: 1.5 days for each two full months of continuous service up to a maximum accumulation of 120 days for non-exempt employees.
Personal Days: One day for each 6 months of service for non-exempt employees, credited on Jan. 1st and July 1st. Personal days cannot accrue from one year to the next.
