Overview

The Hanover Community Nurse is a new initiative planned to help coordinate health care-related services to individuals and families in the Hanover community, with a focus on home visits, teaching and coaching, and promoting both health and wellness and disease prevention.

Town of Hanover Community Nurse Advisory Committee

Purpose: To provide oversight and support of the community nurse

Advisory Committee functions:

  • Establish program mission, vision, and goals
  • Program evaluation process occurs through quarterly and annual review of the data from the Community Nurse Connection
  • Develop an evaluation of services provided by the nurse
  • Review and interview candidates for the town nurse position and make recommendations to the Town Manager
  • Assist the nurse in engaging the community
  • Develop a marketing strategy for the town Nursing program

Membership:

At least 5 community members and representation from the Town administration, Fire and police and aging resource.  Members are volunteers with no term limits
